Software Forms

  • The following forms should be attached to all software purchase orders that are submitted. Please choose the appropriate form that meets your needs.

    Please note: You must be logged into your Alief Google account to access the forms listed below.

     

    Alief Software Approval Form

    • Use this form to request software that has never been used before in Alief I.S.D.
    • Print the form and fill it out completely.

    Alief Software Renewal Form

    • Use this form to request software that has been used before, whether on your campus or another Alief I.S.D. campus. 
    • Print the form and fill it out completely.

    Alief iPad App Evaluation Request Form

    • Use this form to request an iPad app that is not currently on the Approved Digital Tools List.
    • Check the Approved Digital Tools List before you complete the application form.
    • Print the form and fill it out completely.
    • Complete the form with the assistance of your campus Instructional Technology Specialist or Library Information Specialist.
    • Completed forms should be submitted to the designated Technology Integrator for your campus.

    Alief Integration Requests

    • Use this form if your purchased software requires a data connection to the student information system OR requires the creation of an Alief HUB! App.
