The following forms should be attached to all software purchase orders that are submitted. Please choose the appropriate form that meets your needs.
Please note: You must be logged into your Alief Google account to access the forms listed below.
Alief Software Approval Form
- Use this form to request software that has never been used before in Alief I.S.D.
- Print the form and fill it out completely.

Alief Software Renewal Form
- Use this form to request software that has been used before, whether on your campus or another Alief I.S.D. campus.
- Print the form and fill it out completely.

Alief iPad App Evaluation Request Form
- Use this form to request an iPad app that is not currently on the Approved Digital Tools List.
- Check the Approved Digital Tools List before you complete the application form.
- Print the form and fill it out completely.
- Complete the form with the assistance of your campus Instructional Technology Specialist or Library Information Specialist.
- Completed forms should be submitted to the designated Technology Integrator for your campus.
