    Diabetes America will cease all clinical care at all of its locations at the end of business effective May 5, 2017.
     
    Any questions about your medical records should be directed to UT Physicians, the medical records custodian, at 888-4UT-DOC (888-488-3627) or go to www.UTPhysicians.com/DiabetesAmerica.  For each of you, the timing of your next visit with a diabetes Provider will differ.  Some of you will need careful follow-up soon and others may be comfortable to wait for weeks or months.  Whatever the nature of your particular condition, it is important that you continue to get proper and timely care.  You have the right to request to see any diabetes Provider of your choosing, whether in Plano or elsewhere.  You may transfer your care to any physician of choice or you may request a copy of your records be provided directly to you.  Additionally, until you’ve chosen a new diabetes provider, for your medical care you may reach out to:

    ·         Your Primary Care Physician

    ·         UT Physicians at (888)-4UT-DOCS (888-488-3627)

    ·         The Dallas County Medical Society for a list of diabetes providers,

    ·         Your insurance carrier for an approved list of providers, 

    ·         Your local urgent care clinic

    ·         UT Physicians 24/7 Nurse Triage Line 844-880-4103

    ·         For any medical crisis or exacerbations, please immediately go to the nearest Emergency Room.

    You may arrange for the transfer of your medical records to your new healthcare provider.  or your new healthcare provider can request medical records at your first visit.  Please notify UT Physicians at 888-4UT-DOCS (888-488-3627) or go to www.UTPhysicians.com/DiabetesAmerica to obtain forms for transfer of care and/or medical records release.

    Our main goal is to help promote patient care during this transition period.

    Curant Health, our pharmacy partner, is committed to continuing to provide outstanding medication management support for patients:

    ·        If you are a Curant Health patient today, your prescription refills for the next 90 to 180 days have already been sent to Curant Health.

    ·        If you are not a Curant Health patient today, Curant Health staff will be on site to answer your questions about personalized medication management during your upcoming appointment OR you can visit dia.curanthealth.com to learn more about their program.
